Wellington-Dufferin-Guelph Public Health has partnered with Wellington Catholic District School Board to conduct a survey across the district. All students in Grades 4 through 12 will be asked to complete this survey between November 11-15, 2019.
If you do not agree to allow your child to participate in the survey, please fill out the attached refusal form WHY Parent Information Letter and Refusal Form - WCDSB.pdf and return it to the school by November 11th, 2019.
In addition to the student survey, there is also a parent survey that asks about your family’s well-being and experiences with your child’s school. This survey is open throughout the month of November. Please consider completing this survey at:
